Many treatment plans include medications, which can very quickly get overwhelming to keep track of. We help avoid non-adherence by adding prescription tracking and usage history to personal health records and educating each family member on what their specific medication does, what it is called, what it looks like, and how to keep your personal health record up to date with this information.

Personal Health Portfolio (PHP)

We help you gather and manage everyone in your family’s health information in one easily accessible, and secure location, reducing duplicate procedures and potential medical errors.

Cancellations
We are happy to work with your schedule to ensure you are receiving the health advice you need when it is most convenient for you. If needing to cancel a virtual session, we require 24-hour notice and if canceling an in-person session, we require 72-hour notice.

All our consultations are private and confidential. Health and wellness can often be sensitive topics to discuss, so we strive to ensure that you feel comfortable and safe sharing your concerns with us.
